Santé of Chandler has been amazing! The Case managers Ismael, Deb, Guirlie and Luisa have been outstanding in assisting with safe discharge plans and always go above and beyond in communicating next steps of my loved ones. I never have to worry about the safety of patients when in their care. I would highly recommend this facility and give it 10 stars if allowed.

My Uncle was admitted here for a week for treatment. It was such a pleasant experience with wonderful people who were always patient and super helpful. The only concern I had while there was I had several questions about my Uncle's treatment that I was struggling to understand. The Case Manager, Ismael, came in to help and sat down with my Uncle and I and spent almost an hour explaining everything start to finish and answering all of my questions. When he left I felt much better about the direction of my Uncle's care and what the plan for him was. Thank you to all the amazing staff and thank you to Ismael!

I was so impressed with the staff here! They really go above and beyond for their patients. When my dad was in recovery, Jessica came in to fix the air conditioning and asked about how he was doing. At that point, all he could talk about was how much he wanted donuts haha! The next day, Jessica came in to check in on us and had brought my dad a small box of donuts. I could not even believe that she remembered such a small detail and that she cared enough to bring him something. I cannot speak more highly about everyone here. Also a special shout out to our care manager Luisa who was amazing and made sure my dad was comfortable. Thanks again to everyone! You made a really difficult time in our lives a little better ♥️

My father was a patient at Santé of Chandler and he had a great experience while there. He needed quite a bit of equipment when he discharged and his case manager Luisa made sure we had everything we needed before he left. It made everything so much less stressful knowing we were completely prepared at home. We can’t thank her enough!

I have had the pleasure of working with many residents who have discharged from Sante of Chandler and assisting case manager- Ismael. My clients have loved the experience they had here. I’ve heard praises about the therapists, nurses, and the dining staff. It’s a beautiful, upscale, clean facility if you are looking for rehabilitation!

Sante of Chandler is a very well managed facility exhibiting patient centric care. I work closely with the care management team including Debbie, Ismael, Luisa & Girlie. They are professional and easy to coordinate with for discharge care & services for their patients. Each one is truly dedicated & empathetic to their patients’ needs.

Very positive experience at Sante Chandler. When transferred there after a week in the hospital I could not even sit upright. At the end of 5 weeks I was almost back to normal thanks to the well trained, helpful and friendly staff. A special thank you to Janessa of Occupational Therapy and Tiffany and Kathy of Physical Therapy for keeping me on track to reach my goals. HAPPY NEW YEAR to all I
